Please leave the name and phone number of your dog's regular veterinarian, as well as the name and number of an after-hours emergency veterinary clinic if applicable .

Write down the names and dosage information for all of your dog's medications if any.

Non-refundable Deposit required $50 at time of booking . Must reserve appointment  at least 7 days or more  to avoid extra $25 late fee.

If you are late please call within 1 hour  of your appointment time to let us know that you are late. Otherwise your appointment will be cancelled ( any payment besides the deposit will be refunded and you will have to make a new appointment.

No call no show will will result in your appointment being cancelled and future booking restrictions.
